Monday to Sunday, 37.5 hours per week

Job Summary:

To actively participate in the provision of a high quality, effective and efficient Primary Care Service. The post holder will ensure that high standards of service and care are maintained in accordance with the Company ethos and practice. The post-holder will also carry the responsibility for the assessment of patients’ fitness / lifestyle needs and for developing, implementing and evaluating programs of care that will directly impact their health and wellbeing.

 

Key Responsibilities:

  1. To set and maintain high standards of personalised patient care.
  2. To comply with legislative and Company policies and procedures.
  3. Ability to develop and maintain policy and procedure.
  4. Proactive in supporting senior staff.
  5. Ability and willingness to keep up-to-date of new developments within a growing industry.
  6. Ability to work in collaboration with other clinicians.
  7. Awareness of governance issues that reflect safe practice and reduction of any potential risks related to telephone advice.
  8. Adherence to privacy, confidentiality, informed consent, and security policies.
  9. Ability to tailor communication to the individual patient needs.
  10. The post holder will also be expected to work with other departments within the group to assist in future developments and growth of the Company.
  11. The post holder will have excellent written and verbal communication skills, including proficient computer skills.
  12. They must ensure all clinical areas within Primary Care services are adequately stocked and maintained.
  13. Work autonomously in assessing patients, planning, implementing, and evaluating evidence-based care.
  14. Help develop and maintain high standards of service.
  15. Teach / train others in the service including Nursing Staff, HCA’s and new consultants when required.
  16. Responsible, in conjunction with the finance department, for ensuring that pricing for Primary Care department, including researching the service, equipment required and clinician requirements, are achievable, cost effective and in keeping with company ethos.
  17. Delivery of a range of Health Assessments; actively working with patients to identify lifestyle changes that will directly impact their health and wellbeing.
  18. Performance of all clinical tests as required for each specific Health Assessment in accordance with operating protocols.

Qualifications / experience

Registered Nurse

Degree qualified with a minimum of 5 years post registration experience

Live registration with NMC

Experience of working within GP / treatment room / practice nurse setting

Independent Nurse Prescriber (not essential)

Special skills / abilities

Knowledge of current standards within the service.

Can work autonomously in assessing, planning, implementing, and evaluating care.

Effective clinical decision-making.

Reflective safe practice.

Ethical use of information.

Policy and procedure development experience

IT and Telephone interaction skills

Proficient at telephone assessment and advice

Training

Willingness to undertake training and development, when necessary

 

Personal Attributes

Enthusiastic and willingness to learn new roles and take on responsibility as the role develops

Excellent communication skills

Flexibility

Willingness to develop within the role and provide support for new and innovative thinking in healthcare

Confidence in autonomous practice (where appropriate)
